AUA 2025: Robotic Surgery and Patient Safety: A Global Assessment of Technological Malfunctions During Da Vinci X and Xi Procedures

(UroToday.com) Dr. Marco Paciotti evaluated the occurrence of intraoperative technological issues involving the da Vinci Surgical Systems. With the push towards less invasive techniques for improved patient outcomes and surgical efficiency, the number of robotic surgeries being performed has rapidly increased.

With this, we see the concurrent advancement and increased usage of robotic surgical instruments and systems such as the now widely used da Vinci Surgical System by Intuitive Surgical. Although the da Vinci Surgical System is strongly established and accepted for robotic surgeries, there are concerns with the reliability and quality of new robotic platforms of which Dr. Paciotti addressed by assessing the da Vinci X and Xi platforms.

Dr. Paciotti performed a cross-sectional analysis of all robotic procedures involving da Vinci X and Xi throughout the world in 2022 using data collected from Intuitive Surgical’s database. The total number of procedures performed, System-generated Customer Messages (SGCM) troubleshoot alert count, procedure abort count, and conversion to laparoscopic/open surgery count were collected. 

1,835,192 procedures that utilized the da Vinci X or Xi system were included in this study. Dr. Paciotti found that 97.55% of these cases concluded without any SGCMs, while 2.45% of these cases had one or more SGCMs. Procedural abortion and conversion occurred in 0.07% and 0.05% of all cases, respectively. 

Dr. Paciotti concludes his presentation with the affirmation that the da Vinci X and Xi systems are safe and reliable, establishing them as comparative benchmarks for new robotic platforms.

One of the moderators, Dr. Timothy Daskivich, closed the presentation, commenting that the remarkably low numbers of procedural abortions and conversions are reassuring, serving as a testament to the reliability of the da Vinci X and Xi systems for performing robotic surgeries.
